Registration Forms for First Communion (Gr. 2) and Confirmation (Gr. 7)

~ message from Blessed Sacrament Parish

Greetings in this Thanksgiving Season,

Please find the attached welcome letters to First Holy Communion and Conformation programs. Every Grade 2 and Grade 7 student who is baptized is eligible to sign-up. Students in older grades may also register for the sacraments as some students may have missed the opportunity in the past years.

The final date to submit registration forms is December 6.

There are 3 pages- Letter to the parents/guardians, Schedules for the Sacraments and Registration Form in PDF format that is fillable. For the convenience of candidates and their parents/guardians we also attach a separate registration form that can be either printed and filled out or filled and forwarded over the internet. Please note that a copy of the student’s baptismal certificate must be submitted with the registration form.
